As a result of fear of the dentist, the patient continuously delayed treatment of the two teeth and wore a temporary denture for years. The treatment plan envisaged an implant in position 21 and a crown on 11. Since the incisive papilla was directly in the implant region, cone beam tomography was performed in order to clarify the position of the incisive canal. Tooth 11 had been restored with an old metal-supported ceramic crown that suffered from repeated loss of retention, poor fit and unsatisfactory esthetic appearance. 	Download full clinical case as PDF The 77-year old male patient, a nonsmoker in good general health, presented to the Dental Prosthetics Polyclinic of Ludwig Maximilian University in Munich. In the maxilla he wore an inadequate telescopic prosthesis on teeth 21, 22 and 23, which had undergone root canal treatment, and an interim prosthesis in the mandible (43, 42, 41, 31, unrestored natural teeth). The previously inserted implants in region 12, 24 (Straumann® Standard Plus, Ø 3.3 mm, Narrow Neck, length 14 mm, SLActive), 34, 32 and 44 (Straumann® Standard Plus, Ø 4.1 mm, Regular Neck, length 12 mm, SLActive) had not been restored. For the patient, durability and the possibility for extension were essential requirements of the new restoration. Since he wanted a palate-free restoration, further implants were planned to increase the number of abutments. Because of the limited transverse bone, only two further implants could be placed in regions 11 and 45 (Straumann® Soft Tissue Level Standard Plus, Ø 4.1 mm, Regular Neck, length 10 mm). Extensive augmentation was not a possibility for the patient.

	Download full clinical case as PDF A 46-year old patient, a nonsmoker in good general health, presented with an anterior gap in region 21. This was the result of anterior tooth trauma at the age of 12 years. Tooth 21 had been extracted 12 months previously because of persistent inflammation after insufficient endodontic treatment and two endosurgical procedures. The patient refused a bridge and a removable denture was not possible for occupational reasons (concert flautist). Clinical examination showed the following features: adult partial dentition with healthy periodontium, caries-free, small acrylic fillings in the adjacent teeth 11 and 22 and region 21 restored with a customized acrylic denture secured with adhesive to the adjacent teeth 11 and 21. The high laugh line proved unattractive (Fig. 1). The alveolar ridge had a defect of the hard and soft tissue with a vertical and large horizontal component. Plaque removal by the patient was good. Occlusion and laterotrusion were normal (Fig. 2)

	Download full clinical case as PDF Initial situation: The patient presented to my practice for the first time in June 2002. The existing restoration had taken place elsewhere and had been completed in December 2001. The reason for her attendance with me was the dark lower anterior teeth and chips in the new restoration (Figs. 1, 2). There was also increasing discomfort at tooth 23, and the crowns at 17 and 24–27 were about 20 years old and insufficient with marginal caries (Figs. 3 4). After X-rays were taken, it was apparent that tooth 23 had sustained apical damage during the implant treatment (Fig. 5). Despite endodontic treatment, the tooth finally had to be removed.
